STEP 3 BOOK YOUR APPOINTMENT

In accordance with New York Department of Health recommendations to protect our more vulnerable populations, we will not be able to work with clients with compromised immune systems, clients aged 65 or above, or clients in other elevated at-risk categories at this time.

OUR BUILDING

  • A face mask, temperature scan and ID will be required for entrance in the building.
  • Only clients with confirmed appointments will be able to enter the building.
  • If booking a Tween appointment, please list both names in appointment notes when booking online so we can accommodate.
  • Only 3 people will be able to enter the elevator at one time.
  • All common touch points, areas and restrooms are disinfected throughout the building day and night. 

BOOKING YOUR APPOINTMENT

  • ONLINE BOOKING: We will only be accepting appointments that are booked online.
  • CARD ON FILE: To be as touchless & contactless as possible, we require all services to be paid via credit card or EGift Card on file.
  • FOR ALL CLIENTS
  • COVID -19 CONSENT FORM: All clients must submit 48+ HOURS before every appointment.
  • This is to insure that we have this on file if requested by our front lobby security and the NY Health Department. This form is linked below. If we do not receive a Covid Form 48 HOURS BEFORE your appointment, your appointment will need to be rescheduled. 
  • Please reschedule if you or someone in your household is feeling unwell or has been sick. A cancellation fee will not be charged. 

CANCELLATION POLICY

  • If a cancellation is due to illness or weather, we will not charge a cancellation fee.
  • Due to intense demand, all no shows and appointments cancelled within 24 hours notice will be charged the full service amount.

SALON SAFETY

  • Only 1 person/client will be allowed in the salon at one time. 
  • Hand sanitizer is readily available in the salon at all times.
  • Appointments are scheduled 10 minutes apart to create as much social distancing as possible.
  • All contact points, surface areas and stations are cleaned and disinfected after every client.
  • All tools, brushes, and service items/tools are fully sanitized and disinfected with Barbicide and/or UV light.
  • Disposable items/tools are discarded after each and every client.
  • All team members will be adhering to safety protocols as well as social distancing.
  • I will be wearing a mask, gloves and goggles as are mandated by the State Board.
  • I am also fully certified by 2 Associations in new sanitation practices and procedures.

YOUR APPOINTMENT TIME

  • Please do not come in early for your appointment. We ask that you wait outside the building until your scheduled time. 
  • At your scheduled time, please text us to let us know you are at our door. TEXT 917 475 6845

PRODUCTS & PURCHASES

 
  • All retail products will be stored to avoid contact.
  • Testers will not be available at this time.
  • Retail products will be available for purchase by request. All purchases will be shipped to you at no charge.
  • You may purchase products on our website with our free ship code “FREESHIP“.

CHECKOUT

 
  • To save time you are welcome to confirm payment for your appointment before your service starts.
  • We ask that tips be paid via credit card on file instead of cash. 
  • All credit card transactions will be done contactless to avoid touching payment screens as well as signature not being required. 
  • To keep our team safe, please let us know if you show any apparent COVID-19 symptoms after your appointment.

COVID-19 INFORMATION & CONSENT FORM

COVID-19 is a highly contagious virus that spreads from person to person. In addition to long-held and explicit sanitation measures this business has always adhered to, new preventative measures have been put in place to further reduce the spread of this novel coronavirus. However, these best practices still offer no guarantee regarding your potential risk of being infected. Please answer the following in order for your appointment to be confirmed.

In exchange for being permitted to enter the premises of Elke Von Freudenberg Salon (the “Business”), located at  1140 Broadway, Suite 906, New York, NY, 10001 (the “Premises”) to Personal Services (the “Activity”), I agree to the following as described in this contract.

I will follow all of the instructions of the Business while on the Premises, including: protocols set forth by our building, and following all salon protocols i.e. wearing a mask at all times.
I agree not to enter the Premises if I am experiencing symptoms of COVID-19 such as such as any respiratory or flu symptoms, sore throat, or shortness of breath , have a confirmed or suspected case of COVID-19, or have come in contact in the last 14 days with a person who has been confirmed or suspected of having COVID-19.

I am aware of the highly contagious nature of COVID-19 and the risk that I may be exposed to or contract COVID-19 by being on the Premises and engaging in the Activity.
I acknowledge that I am voluntarily entering the Premises to engage in the Activity with knowledge of the danger involved. I hereby agree to accept and assume all risks of personal injury, illness, disability, or death related to COVID-19, arising from my being on the Premises or engaging in the Activity, whether caused by negligence of the Business or otherwise.

I hereby expressly waive and release any and all claims, now known or hereafter known, against the Business, or owners, staff and service technicians on account of injury, illness, disability, or death arising out of or attributable to my being on the Premises or engaging in the Activity and being exposed to or contracting COVID-19, whether arising out of the negligence of the Business, owners, staff and service technicians , or otherwise.

on the planned date of my appointment, I will cancel my appointment if:
I understand that I will need to sanitize my hands upon arrival and wear a face covering in the salon at all times. I understand that, because esthetics involves maintained touch and close physical proximity over an extended period of time, there may be an elevated risk of disease transmission, including COVID-19. I understand that my practitioner will be practicing precautions by wearing a mask and gloves throughout my service. By signing this form, I acknowledge that I am aware of the risks involved from receiving service at this time, I voluntarily agree to assume those risks, and I release and hold harmless the practitioner/business from any claims related thereto. I give my consent to receive treatment from this practitioner.
